What major league team had the longest streak of the same person throwing out the ceremonial first pitch for Opening Day??

Not positive but I think it's the Braves

Not the Braves.
BTW, to state the obvious you should identify who the person is who threw out the ball and roughly the time frame.

I would guess that FDR threw out the opening pitch from 1933 to around 1940.

It was not a President.

How about Charlie Bennent. He was a good hitting and good defensive catcher until he was involved in a train accident in 1894 and had to have both legs amputated. He was returning from a hunting trip when he got off the train in Wellsville, Kansas to speak with an acquaintance. When he tried to reboard, he slipped and fell under the train. He is also credited with inventing the chest protector. The Tigers first home, Bennent Park, was named in his honor. He threw out the pitch for the Tigers from 1901 to 1926. He passed away in February 1927.
However I noticed on his wikipedia page that "Bennett caught the first pitch at Bennett Park in 1896. It became a Detroit tradition for Bennett to catch the first pitch in Detroit, an honor that Bennett continued for every home opener through 1926."
So whether he threw it or caught it, he did it for 26 years.

It was Charlie Bennett. Baseball-Almanac has him throwing out the first pitch. I don't know, literally, which is right, but I suspect catching would be difficult to do without legs.

Well, it did say he was fitted with artificial limbs so it is possible he caught the first pitch. However, wikipedia is not always the most reliable source.
