I know this is a very watered down and generic question, but what are the main reason(s) somebody will swing and miss the ball (assume only fastballs). And why does it look like those strikes the bat is missing below the ball? This seems to be consistant from LL through the big leagues.

From this relative non-entity, I always seem to swing late, not good at judging the speed of pitches.

I once ran into this with a guy who was striding too far forward (e.g. > 12") and sliding under the ball.
