Pitcher Y enters the game to pitch to Player A with Player B on first, Player C on second, and Player D on third. One out later Pitcher Z comes in to pitch to Player A with Player B on first, Player C on second, and Player D on third.
Explain on this happened and the 6 players/pitchers involved.

My guess is Player A batted out of turn, did something productive, and when it was protested he batted out of turn the batter who was supposed to be up was called out and all runners had to return to their respective bases. Thus the next batter comes to the plate, which, in this case, just happens to be Player A again.

Hint: The case I describe actually did happened and one of the players involved was Will Clark.

Hint 2: One of the "pitchers" last name was Alexander.

Did the team bat around, only using up one out in the process? That must have been one big inning, since a minimum of eight runs have scored already, and the bases are still loaded.

The "pitcher" mentioned earlier has the first name Manny and he is pitcher B. Should be easy to figure out all of the details now.

Another Hint: Pitcher A was Jesse Orosco. Pitcher B was already given as Manny Alexander.

Hamilton on 3rd, Valle on 2nd, Clark on 1st, Orosco comes in to pitch, relieving Benitez.
Orosco gave up a sac. fly, a walk, and 6 hits, and is replaced by (none other than) Manny Alexander, with Hamilton on 3rd, Valle on 2nd, and Clark on 1st.
Alexander gave up 4 walks, one hit (HR), and 2 outs (one on a sac fly).
16 runs scored in the inning, on 8 hits.
Final score: 26-7

Orosco entered the game with the bases loaded and Juan Gonzalez coming up and left one out later with the exact same runners on base and Juan Gonzalez coming up again.
The Orioles then brought infielder Manny Alexander to pitch. At that time the Rangers also made a move sending in Damon Buford to pinch run for Will Clark.
